Immunizations
The South Carolina Department of Health and Environmental Control requires specific vaccinations for students based on their age and grade. It is very important to check with your doctor and make an appointment for your child to receive any required vaccinations. Proof of immunizations must be provided to the school, and forgetting to schedule vaccination may prevent your student from returning to school on time at the beginning of the year.

Where do I take my child to get shots required for school?
You can go to your personal medical provider, clinics, or to one of the local Clinic & Health Departments listed below. Please note that SC DHEC Immunization Clinics are by appointment only and book up quickly. Please schedule an appointment with your child's health care provider or call DHEC in advance for an appointment. To schedule an appointment at a DHEC clinic, patients must call the DHEC appointment line at: 1-855-472-3432.
Field Trips
Notify teacher and nurse if your child has a special health care need. The health care provider must complete the Parent Information For Medications and Medical Procedures & Medication / Procedure Doctor’s Orders in order for the student to have medication at school and on field trips. Self-Medicate and Monitoring Doctor's Orders are only for life saving medications/procedures such as insulin, asthma inhalers and epi pens.
Over the Counter Medications (OTC) will not be provided on field trips without a completed Parent Information For Medications and Medical Procedures & Medication/Procedure Doctor’s Orders.
Self-Medication Orders are not valid for over-the-counter medications. OTC medications should be provided by the parent in a manufacturer's container with the seal and label intact.
Medication administration will be the responsibility of the trained designated CCSD employee.
